Our immunisation services include:
- A free childhood immunisation program run in line with the National Health and Medical Research Council's childhood immunisation schedule.
- A school immunisation program for Year 7 and Year 10 students.
- A monthly immunisation program operates from five venues in Moonee Valley.

School Entry Immunisation Status Certificate
You must provide a school entry immunisation status certificate on enrolment to primary school in Victoria.
Your child’s immunisation history statement, supplied by the Australian Childhood Immunisation Register, can also act as a school entry immunisation status certificate.
This statement will be automatically posted out to you once your child has completed the four year-old immunisations and has completed all required childhood immunisations.
The statement should have the following printed on the bottom of the page:
“This child has received all vaccines required by 5 years of age”
You can now log in and access your child's Immunisation History Statement on the Medicare website.
You can request for a statement to be posted to you by calling Medicare on 1800 653 809.

Application for Immunisation Records (Prior to and including 1998)
Our Enviromental Health Unit can send you your immunisation records if you have been immunised through our immunisation service.
We cannot provide records of immunisations given by doctors or other medical practitioners.

You can call the Australian Childhood Immunisation Register (ACIR) on 1800 653 809 for immunisation records of children under seven years old.
Fees and procedures
- There is a $20 search fee to retrieve records prior to and including 1998.
- Please allow 10 working days for us to find the records. Once they are found they will be posted to you.
- The $20 search fee is non-refundable regardless of search results.
