Home care packages

We’re proud to offer a reliable, high-quality service to older residents in our community through Home Care Packages.

A Home Care Package is a coordinated package of care and services to help you live independently in your own home for as long as you can.

Home Care Packages are about empowering you to take control of your life with choice and flexibility.

What types of packages are available?

There are four levels of Home Care Packages which are designed to give you the care you need. A professional assessor from the Aged Care Assessment Service (ACAS) will work with you to find out which home care package is appropriate for you. Each level provides a different subsidy amount and is paid to the home care provider you choose.

  • Level 1 supports people with basic care needs
  • Level 2 supports people with low-level care needs
  • Level 3 supports people with intermediate care needs
  • Level 4 supports people with high-level care needs

Services and support available under Home Care Packages program?

A wide range of care and support services is available so that you can remain living independently in your own home and achieve your goals. Your home care provider will help you choose and coordinate the services appropriate for your needs.

  • Case Management: A Case Manager will be allocated to you to be your one point of contact. They will discuss your needs and preferences with you, develop a goal-directed care plan and work with you to get the most out of your package.
  • Personal services: assistance with personal activities such as bathing, showering, toileting, dressing, mobility and communication
  • Domestic assistance: assistance with cleaning, laundry, shopping, meal preparation
  • Transport and personal assistance: assistance with shopping, visiting medical appointments, attending social activities
  • Management of continence, skin integrity and medication
  • Allied health and nursing services, such as physiotherapy, speech therapy and other
  • Assistive technology
  • Aids and equipment, for example walking aids, bathroom modifications and other

Home Care Package funds cannot be used as a general source of income for items such as day-to-day bills, food, mortgage payments or rent as an example.

How do I find out whether I'm eligibile?

To become eligible for a Home Care Package, you need to phone the My Aged Care contact centre on 1800 200 422. You can also visit the My Aged Care website.

My Aged Care will register you and conduct a screening of your needs. If your care needs indicate you might require a Home Care Package, you will be referred to the Aged Care Assessment Service (ACAS) for a face-to-face assessment. The assessment will determine if you are eligible for a package and if so, what level. You will be placed on a national prioritisation system and receive a letter once you are assigned a Home Care Package. The wait for a package might be several months. You can choose who your home care provider is, from a range of options including Moonee Valley City Council.

If you find that you are not eligible for a home care package, you may still be able to access some of our services through the Commonwealth Home Support Program (CHSP) (link to “In-home support for people over 65.

Cost

Depending on your financial circumstances, you may be required to contribute to the costs of your care and services. This is called an income-tested care fee. At Moonee Valley City Council, we do not charge a contribution to people who only receive the full aged pension.

We will discuss the costs and fees in detail with you when you contact us, and provide you with a written budget.
